  • UNEMPLOYMENT: Form-1099 G is REQUIRED. Visit your state’s Department of Labor website to obtain accurate information, including withholding.

  • MAIL (INCLUDING FED-EX, UPS, USPS EXPRESS): Please mail tax preparation documents to our office address at the bottom of our website.
